Job Title: Home Care RN

At Trinity Health, we are committed to providing exceptional care to our patients in the comfort of their own homes. As a Home Care RN, you will play a vital role in delivering high-quality, patient-centered care to our patients.

About the Role:

As a Home Care RN, you will be responsible for providing primary nursing care to patients in their homes, as ordered by the physician and in accordance with our organization's policies and procedures. You will work closely with our Population Health Case Manager to develop and implement care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide one-to-one care to patients in their homes, focusing on their physical, emotional, and social needs.
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans.
  • Supervise and support Home Healthcare Aides and LPNs as needed.
  • Provide therapeutic interventions and education to patients and their families.
  • Monitor patient outcomes and adjust care plans as needed.
Requirements:
  • Graduate of an approved nursing education program.
  • Licensure as a Registered Nurse in the state of PA.
  • One year of experience as a professional care nurse.
  • Current Driver's license and reliable transportation.
  • Ability to work a weekend rotation once monthly and 2 holidays yearly.
