Job Summary

Key Responsibilities
  • Develop and implement individualized discharge plans to meet patient needs and promote efficient utilization of resources.
  • Identify at-risk populations and facilitate safe transitions to next-level care to prevent readmissions.
  • Collaborate with the interdisciplinary team, patients, and families to achieve optimal clinical outcomes.
  • Practice autonomously, consistent with evidence-based standards, and serve as a case management resource expert for compliance with regulatory requirements.
  • Work with utilization department, physician advisors, and other stakeholders to ensure appropriate and cost-effective care.
  • Evaluate patient daily plans for effectiveness and involve patients and families in formulating plan goals.
  • Maintain active communication with physicians, nursing, and other members of the interdisciplinary care team to effect timely and appropriate patient resource management.
Requirements
  • Current, valid State of Florida RN license or endorsement.
  • At least three (3) years of clinical experience in an acute care hospital.
  • Strong analytical, data management, and computer skills.
  • Excellent interpersonal communication and negotiation skills.
  • Demonstrated critical thinking in resolving or escalating barriers.
Preferred Qualifications
  • BSN or Bachelor's Degree in related field.
  • CCM or ACM certification.
  • Case Management experience in an acute care hospital.
  • Knowledge of care transitions, utilization management, case management, performance improvement, and managed care reimbursement.
Work Environment

The RN Case Manager- Case Management role is a sedentary position that involves sitting or standing for long periods, with occasional walking, bending, and stooping. The workspace may vary from open to confined, and the role may require travel to various facilities within and beyond the county perimeter.
