The LPN will utilize the nursing process in planning, implementing, and evaluating nursing care to patients, collaborating with other team members to provide health information and supportive care. This role requires the ability to collect data on patient health status, identify health goals, and assure care that reflects our organization's core values.

Key responsibilities include:

  • Implementing medical and nursing plans of care, nursing interventions, medication administration, and procedures necessary for patient care under the direction of the Registered Nurse.
  • Providing direct patient care within the primary nursing model to ensure continuity of care for all patients, maintaining patient rights and dignity at all times.
  • Performing technical aspects of patient care safely, according to established policies and procedures, and Nurse Practice Act.
  • Communicating changes in patient condition immediately to the Registered Nurse.
  • Basing all nursing interventions on pathophysiological and psychosocial rationale, delivering care in a fiscally responsible manner.
