Home Health Speech Language Pathologist

Adventist Health is seeking a skilled Home Health Speech Language Pathologist to join our team in Simi Valley, California.
Job Summary:As a Home Health Speech Language Pathologist, you will provide speech therapy services to patients with speech, language, or swallowing disorders in the comfort of their own homes.
Responsibilities:- Assess and document speech therapy data related to patient assessments, patient care needs, interventions, and outcomes.
- Identify changes and trends throughout physical and emotional parameters.
- Review treatment plans and assess individual performances to modify, change, or write new programs.
- Maintain documentation as required by law, establishment policy, and administrative regulation.
- Instruct patients on ways to monitor speech and practice new skills.
- Perform other job-related duties as assigned.
- Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required
- Master's Degree: Preferred
- Speech Pathologist licensure in the state of practice: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required
- American Speech-Language and Hearing Association (ASHA) Certificate of Clinical Competence (CCC): Required
- Speech-Language Pathology (SLP): Required
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP): Required
