Position Summary:

Key Responsibilities:

  • Conduct outreach to correctional facilities and community organizations to identify and engage eligible clients.
  • Perform intake assessments and develop tailored case management plans that address the unique needs of each client.
  • Facilitate access to social services and resources, ensuring clients receive appropriate referrals.
  • Utilize effective case management techniques, including advocacy, monitoring, and planning for successful reintegration.
  • Organize and participate in case conferences with clients and their support networks to create collaborative care plans.
  • Ensure compliance with all documentation requirements as per state regulations.
  • Travel to meet clients in various community settings and participate in relevant meetings.
  • Attend mandatory training sessions to enhance skills and knowledge in line with organizational policies.
  • Carry out additional duties as assigned by the Director.

Qualifications:

  • Bachelor's degree in social work, human services, criminal justice, or a related field from an accredited institution.
  • A minimum of one year of relevant experience in human services, corrections, or community support.
  • Experience may be substituted for academic qualifications, with a total of five years of combined education and experience required.
  • Possession of a valid driver's license.

Skills and Abilities:

  • Ability to comprehend and adhere to contractual guidelines, policies, and procedures.
  • Strong communication skills, with the capacity to present information clearly to diverse audiences.
  • Willingness to travel as necessary and maintain a flexible work schedule.
  • Capability to work independently with minimal oversight.
