Job Summary:We are seeking a skilled Clinical Assessment Advisor to join our team. The ideal candidate will have a strong background in nursing, preferably with experience in clinical management roles. This position requires excellent communication skills, both verbal and written, as well as the ability to work effectively with healthcare professionals and patients.
The successful candidate will be responsible for completing and overseeing the development of patient plans of treatment, performing ongoing evaluations of patient needs, and coordinating with multidisciplinary teams to ensure optimal clinical outcomes. A strong understanding of state and federal home health agency regulations and compliance standards is essential.
Key Responsibilities:- Complete and oversee the development of patient plans of treatment
- Perform ongoing evaluations of patient needs and coordinate with multidisciplinary teams
- Ensure compliance with state and federal home health agency regulations and standards
- Bachelor of Science degree in Nursing (BSN) preferred
- At least 2 years of clinical home care experience, with a minimum of one year in a clinical management role
- Current CPR certification
- Excellent verbal and written communication skills
- Ability to work effectively with healthcare professionals and patients
