About the Role

Bridgewell is seeking a dedicated and compassionate Direct Support Professional 3 to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care and support to individuals with developmental disabilities.

Key Responsibilities
  • Provide Direct Care and Support: Assist individuals with daily living activities, such as bathing, dressing, and meal preparation, to promote independence and dignity.
  • Implement Individualized Care Plans: Work closely with interdisciplinary team members to develop and implement personalized care plans that meet the unique needs and goals of each individual.
  • Document Progress and Maintain Records: Accurately document progress, maintain accurate records, and communicate effectively with team members to ensure seamless care coordination.
  • Promote Independence and Community Integration: Facilitate recreational activities, outings, and social events to promote socialization, community integration, and overall well-being.
  • Collaborate with Interdisciplinary Team Members: Work collaboratively with healthcare professionals, therapists, and other support staff to ensure holistic care and address the complex needs of individuals.
  • Maintain a Safe and Clean Environment: Ensure a safe and clean environment for residents, adhering to established protocols and guidelines to prevent accidents and promote a healthy living space.
