Job Overview: We are seeking a skilled and compassionate Community Health Advocate to join our Assertive Community Treatment (ACT) team in Bennington County. As a Service Coordinator, you will provide flexible and supportive services to individuals experiencing mental health challenges, homelessness, and substance use issues.

Responsibilities:
  • Engage in relationship building/rapport building with clients and provide service with a client-centered, harm reduction, and strengths-based approach
  • Provide one-to-one support via regular home visits with life skills, housing concerns, and health maintenance
  • Assist clients in navigating the housing path, including viewing units, assistance with moving in, life skill development, landlord/client disputes, etc.


Estimated Salary Range: $40,000 - $50,000 per year based on experience and qualifications.
