Job Summary

We are seeking a highly skilled Substance Use Disorder Treatment Coordinator to join our team at Vera French Community Mental Health Center. As a key member of our interdisciplinary team, you will work closely with outpatient providers to coordinate substance use treatment and mental health services for individuals with co-occurring disorders.

Key Responsibilities
  • Conduct comprehensive drug and alcohol assessments using evidence-based tools, such as ASAM and SBIRT.
  • Engage clients in comprehensive bio-psychosocial assessments to identify gaps in care and provide linkage to appropriate services.
  • Assist clients in identifying and accessing informal and formal supports, including social, recreational, family, spiritual, and neighborhood resources.
  • Help clients navigate formal service systems in the community, including substance use and mental health treatment services, medical healthcare, housing, income/employment, educational, social service, and other related services.
Requirements
  • Good working knowledge of substance use disorder services, community resources, and how to access these resources.
  • Ability to work with a diverse population and effectively collaborate with an interdisciplinary team.
  • Ability to adhere to agency policies of confidentiality, ethics, and mandatory abuse reporting.
  • Bachelor's degree in social work and/or criminal justice or a related field.
  • CADC or ACADC certification required.
  • Previous experience working with individuals with co-occurring substance use and mental illness, with a preference for experience using the ASAM framework.
