Position Overview

The Physical Therapist will evaluate participants, implement physical therapy interventions, and conduct both initial and ongoing assessments. This role includes supervising physical therapy assistants, documenting participant progress, and educating participants and their families involved in the Program for All-Inclusive Care for the Elderly (PACE).


Our community health model allows therapists to provide care in both outpatient settings and participants' homes as necessary. This position collaborates with an interdisciplinary team (IDT) to create and execute individualized care plans, ensuring that participants can maintain independence for as long as possible.

Physical Therapist Pay Range:
$62.22

Qualifications

Master's degree in physical therapy from an accredited institution, or a Bachelor's degree in physical therapy with certification for those who graduated prior to a specified date.
