  • Job Description: As a Nurse Practitioner with Matrix Medical Network, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, and follow-up, all while working collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
  • Responsibilities:
    • Conduct home visits to review and observe a member's current health, medical history, medication adherence, social environment, and other risks.
    • Collaborate with primary care physicians on patient education and provide follow-up care.
    • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
  • Qualifications:
    • Master's Degree or commensurate experience and satisfactory completion of NP licensure.
    • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
    • Board certified by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
    • Current BLS, ACLS, or CPR certification.
    • Minimum of 1 year experience as a Nurse Practitioner.
    • Strong computer skills and familiarity with employee health/medical record software.
    • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Travel Requirements:
    • Ability to travel and a valid state driver's license, with proof of adequate automobile insurance coverage.
    • Travel required to meet patients where they live.
