Senior Care Management Specialist

Join our dedicated team and make a significant impact on the lives of seniors and adults facing physical or cognitive challenges while enjoying the flexibility to manage your own schedule.

WORK HOURS
This is a full-time role with some flexibility. The ideal candidate should be prepared to work on weekends, holidays, or be 'on-call' as needed to ensure client safety. Hours may increase as the caseload expands.

EDUCATIONAL REQUIREMENTS
A Master's degree in Social Work or a Registered Nurse (RN) designation is preferred.

EXPERIENCE
Candidates should be eligible for certification by the National Academy of Certified Care Managers. A minimum of 5 years of RN experience in eldercare is required, while LPN candidates must have at least 10 years of eldercare experience. Experience as an Advanced Registered Nurse Practitioner (ARNP), Director of Nursing, or Charge Nurse is preferred. All candidates must possess at least 2 years of experience in Care Management, Social Work, or Hospice care. Strong written and verbal communication skills are essential. Experience working with individuals diagnosed with Alzheimer's disease or other forms of dementia is necessary. Familiarity with care management programs is a plus.

RESPONSIBILITIES IN AGING LIFE CARE MANAGEMENT
- Conduct health and wellness assessments and provide recommendations.
- Plan, coordinate, and implement individualized care plans for clients.
- Supervise recommendations from assessments and care management meetings.
- Coordinate medical appointments, including physician and dental visits.
- Oversee medical issues, including medication management, and accompany clients to doctor visits.
- Communicate with and support family members and other responsible parties.
- Prepare weekly schedules of planned and completed activities.
- Maintain accurate client records and documentation.
- Participate in the recruitment, orientation, evaluation, and retention of service providers.
- Advocate for clients, including obtaining restraining orders and testifying in court as needed.
- Reliable transportation to client residences is required.

MARKETING AND INTAKE RESPONSIBILITIES
- Handle potential client intake calls to assess needs and align them with appropriate services.
- Attract referrals and inquiries to expand care management services.
- Engage with potential and existing healthcare referral sources on behalf of Aging Life Care Association.
- Promote and position our programs and services to medical professionals and community groups, showcasing our commitment to quality care.

- Must have a complete home office setup, including a smartphone, internet access, fax, scanner, copier, shredder, and a dedicated workspace.

SKILLS AND EXPERIENCE REQUIREMENTS
- Comprehensive knowledge of the aging process, including diagnoses, psychosocial issues, medical procedures, and medications.
- Understanding of the physical, emotional, social, leisure, and spiritual needs of clients.
- Supervisory and well-developed communication skills (both verbal and written).
- Strong interpersonal communication abilities and a thorough understanding of medical terminology.

COMPENSATION
Compensation is based on billed hours, reflecting skills, certifications, and experience.
