Role Summary:

Your primary responsibility is to provide high-quality patient care coordination services. You will engage with your assigned panel of patients to deliver monthly care coordination services, educating them about their conditions and collaborating with their healthcare providers as necessary. Effective communication with patients, their families, and practice staff is essential to develop tailored action plans that facilitate the management of chronic illnesses.

This role requires a compassionate individual with a solid understanding of healthcare practices, capable of working remotely while delivering exceptional patient care coordination services. You will handle a substantial call volume daily, meeting productivity goals as set by your supervisor.

Key Responsibilities:

  • Conduct monthly outreach calls to patients, practices, and pharmacies to ensure their needs are met between office visits, dedicating 7-8 hours daily to phone interactions.
  • Create and update personalized care plans for each patient, collaborating with their healthcare providers.
  • Identify social determinants of health and gaps in care, coordinating with office staff and community resources to address these effectively.
  • Empower patients to take charge of their wellness within the context of their care plans.
  • Make approximately 30-50 calls daily to achieve the necessary patient interactions for care coordination services.
  • Maintain patient privacy and confidentiality, adhering to ethical and regulatory standards at all times.

Qualifications:

  • 1-2 years of healthcare experience as a Certified/Registered Medical Assistant with active accreditation.
  • Proficiency with multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite.
  • Professional demeanor on the phone, with the ability to maintain a compassionate and engaging tone.
  • Strong critical thinking skills to respond to patient inquiries and prioritize their needs.
  • Exceptional business writing skills, ensuring clear and accurate communication.
  • Self-motivated, organized, and capable of working independently.
  • Experience in a call center or triage setting is advantageous.
  • Experience working with a geriatric population is a plus.
  • Background in case management or care coordination is beneficial.

Working Conditions:

  • Able to work a 40-hour schedule, Monday through Friday.
  • A private home office setup is required to ensure confidentiality during patient interactions.
  • Reliable internet connection with a minimum upload speed of 50 Mbps is necessary; satellite internet or mobile hotspots are not acceptable.
