Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. As a key member of our clinical team, you will conduct adult and geriatric assessments, provide medical history, diagnosis, and treatment, and collaborate with physicians, case managers, and other healthcare professionals to deliver high-quality patient care.

Responsibilities
  • Conduct comprehensive assessments, including medical history, diagnosis, and treatment, in a variety of settings, including home visits, skilled facilities, and telehealth visits.
  • Collaborate with physicians, case managers, and other healthcare professionals to develop and implement patient care plans.
  • Provide education and support to patients and their families to promote health and wellness.
  • Document patient information accurately and thoroughly, in accordance with Matrix Medical Network guidelines and protocols.
Requirements
  • Master's degree in Nursing or Physician Assistant studies, or equivalent experience and licensure.
  • Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant, with the ability to travel and work in a variety of settings.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and healthcare professionals.
