Job Title: Psychiatric Nurse Practitioner

We are seeking a part-time Psychiatric Nurse Practitioner to join our Assertive Community Treatment (ACT) Team at Mohawk Opportunities.

About the Role:

The Psychiatric Nurse Practitioner will be a member of our community-based multidisciplinary treatment team, providing psychiatric assessment and treatment to individuals with severe mental illness. The successful candidate will have strong clinical skills and experience working with individuals with complex medical issues.

Key Responsibilities:
  • Conduct person-centered psychiatric assessments, including psychiatric history, course of illness, response to treatment, mental status examination, and DSM diagnoses.
  • Collaborate with nursing staff regarding client medical needs and recommendations for referrals to community physicians.
  • Prescribe psychiatric medications, provide medication education, assess efficacy of medications, and monitor for side effects.
  • Attend morning staff meetings to review client progress and make recommendations about course of treatment.
  • Provide support, education, and counseling to family members of clients as needed.
  • Collaborate with inpatient providers during acute or long-term hospitalization of a client.
  • Participate in and supervise the development of comprehensive treatment plans for each patient.
Requirements:
  • NYS Licensed Psychiatric Nurse Practitioner with board certification preferred.
  • Minimum of three years of experience in psychiatry.
  • Strong commitment to the rights and ability of each person with a mental illness to make their own decisions and choose to live in the community.
  • Valid and insurable driver's license.
