Position Overview

The Advanced Practice Nurse collaborates with a multidisciplinary team to develop, assess, and modify the care plan for patients. This role involves providing direct medical services to patients in their homes and offering guidance to nursing personnel. The Advanced Practice Nurse conducts in-person evaluations under the oversight of the Medical Director.

Reporting Structure

Reports to: Chief Operating Officer (COO)

Collaborates with: Interdisciplinary Team and administrative staff

Key Responsibilities

  • Upholds the mission and values of Patriot Hospice, emphasizing human dignity and a compassionate response to individuals facing life-limiting or chronic conditions.
  • Works alongside the Interdisciplinary Team to formulate, assess, and update the patient's care plan.
  • Delivers medical services within the authorized scope to patients in their residences.
  • Consults with Hospice nurses on patient care matters and collaborates with the Hospice Medical Director on medical issues beyond the Nurse's scope.
  • Provides education and support to patients and their families regarding preventive care and medical concerns as necessary.
  • Conducts home visits to assess patients' ongoing eligibility for Hospice services, including performing necessary evaluations.
  • Records all patient interactions and services rendered in the electronic medical record system.
  • Maintains professional development through ongoing educational opportunities.
  • Fosters and maintains relationships with staff and community referral sources to enhance program outreach.

Qualifications

A valid Nurse Practitioner Certification and state license are required. Experience in management within long-term care or hospice settings is preferred. Proficiency in computer skills is essential.

  • Knowledgeable about hospice nursing practices, medical procedures, and relevant laws and regulations.
  • Possesses leadership qualities and the ability to work collaboratively with others.
  • Ability to plan, organize, and implement care programs and policies effectively.
  • Exhibits patience and tact, especially when dealing with challenging situations.
  • Willingness to explore and integrate new methods into existing practices.
  • Ability to communicate effectively regarding patient conditions.
  • Must not pose a health or safety risk to others in the workplace.
  • A caring and adaptable individual committed to addressing patient needs.
  • Proficient in documentation and computer usage.
  • Knowledge of HIPAA regulations and compliance.
  • Must possess a valid driver's license and auto insurance, with the ability to travel within the service area.
  • Work environment includes office settings, community locations, and long-term care facilities.
  • Must be able to lift and move at least 25 pounds.
