Job Summary

We are seeking a highly skilled Community Based Rehabilitation Specialist to join our team at Northwest Children's Home. As a Community Based Rehabilitation Specialist, you will work in the home of clients and in the community to teach interventions and practice skills to support clients' ability to manage functional needs independently.

The ideal candidate will have a strong background in therapeutic services, including experience with Dialectical Behavioral Therapy (DBT) and Trauma Focused Cognitive Behavioral Therapy (TF-CBT). You will work closely with clients, families, and other healthcare professionals to develop and implement individualized treatment plans.

Key Responsibilities:

  • Provide therapeutic services to clients in their homes and in the community
  • Develop and implement individualized treatment plans in collaboration with clients, families, and other healthcare professionals
  • Utilize evidence-based therapeutic techniques, including DBT and TF-CBT
  • Work closely with clients and families to promote independence and self-advocacy
  • Maintain accurate and timely records of client progress and treatment plans

Requirements:

  • Bachelor's degree in a related field
  • Valid state-issued driver's license
  • At least 21 years of age
