Job Summary

We are seeking a skilled Hospice Nurse to join our team on a PRN basis. As a Hospice Nurse, you will be responsible for providing high-quality patient care and support to patients and their families in their most vulnerable time.

Key Responsibilities
  • Triage of After-Hours Patient and Family Calls: Respond to after-hours patient and family phone calls, determining the need for services including assessments, symptom management, and care plan updates.
  • Assessment and Intervention: Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family needs and initiate intervention by other members of the interdisciplinary team as appropriate.
  • Communication and Coordination: Initiate communication with the attending physician, other interdisciplinary staff members, and other agencies for the purpose of coordinating optimal care.
  • Clinical Pronouncement of Death: Perform clinical pronouncement of death and provide support to family.
  • Clinical Record Maintenance: Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes.
Requirements
  • Education: Graduate from an accredited registered nursing program.
  • Licensure: Possession of a current license to practice in the state as a Registered Nurse.
  • Experience: Minimum of two years of varied work experience as a professional nurse.
  • Clinical Skills: Demonstrated skill in nursing practices and clinical judgments.
  • Interpersonal Skills: Ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Certifications: Must possess current CPR Certification.
  • Driver's License: Valid driver's license and proof of insurance is required.
