Main Responsibilities
This role involves advising patients and family caregivers on community resources and supports, guiding them through the process as needed. As part of our inpatient care management team, the successful candidate will work closely with healthcare professionals to deliver high-quality care.
Requirements
To be successful in this role, you will need a Bachelor's Degree in Social Work and current licensure as a Licensed Social Worker in Maine. Previous hospital social work experience is preferred but not required.
