About the Role

Goshen Health is seeking a highly skilled Licensed Social Worker to join our team as a Patient Experience Specialist. In this critical role, you will be responsible for ensuring the exceptional experience of care for our patients and their families.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments and develop clinical interventions to address complex care needs.
  • Provide short-term therapeutic interventions, crisis management, and resource development to improve patient flow and transition across the continuum.
  • Coordinate care across the continuum, collaborating with physicians, continuity of care planners, and other interdisciplinary team members.
  • Identify and address barriers to care, including social, financial, and environmental factors that impact patient outcomes.
  • Engage patients, families, physicians, and the healthcare team in the transition plan of care to achieve better alignment with goals, department priorities, and system quality initiatives.
  • Perform other related duties as assigned to support patients across the continuum of care within our healthcare setting.
Requirements
  • Permanent LSW licensure is required.
  • LCSW licensure is preferred.
  • A Master's Degree in Social Work is required.
  • Relevant healthcare, mental health, or community-based experience is required.
  • Knowledge of the impact of disease processes and human behavior is necessary to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention.
  • Strong clinical assessment, planning, and goal development skills are essential.
  • Ability to communicate and resolve conflicts with diverse stakeholders, including physicians, community agencies, clients, and families.
  • Knowledge of community resources and entitlement programs.
  • Knowledge of state and county adult and child protective legal mandates.
  • Knowledge of the principles of growth and development over the life span.
  • Strong communication and counseling skills to develop positive helping relationships with patients, families, physicians, and other healthcare team members.
