As a social worker at The Laurels of Goshen, you will play a vital role in providing medically-related social services that help our guests attain and/or maintain their physical, mental and psychosocial well-being.

Job Summary

We are seeking a highly skilled and compassionate social worker to join our team. As a Director of Social Services, you will be responsible for providing social services evaluations, referrals, and communication with residents and their families. You will also assist guests with financial and legal matters, provide discharge planning services, and ensure accurate and timely documentation in MDS care plans.

Key Responsibilities
  • Conduct social services evaluations and provide referrals to residents and their families.
  • Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements.
  • Provide discharge planning services, including helping to place residents on waiting lists for community congregate living, arranging intake for home care services, and assisting with transfer arrangements to other facilities.
  • Provide or arrange needed counseling services and psychiatric medication management as indicated.
  • Ensure accurate and timely documentation in MDS care plans.
Requirements
  • Current social work licensure in the state preferred, but not required.
  • Bachelor's degree in social work, psychology, or human services field required.
  • One year of social work experience in a skilled nursing facility setting is preferred, but not required.
