Job Summary

Goshen Health is seeking a skilled Clinical Social Worker to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring a seamless transition across the continuum of care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
  • Provide short-term therapeutic interventions, crisis management, and resource development to improve patient outcomes.
  • Collaborate with physicians, continuity of care planners, and other interdisciplinary team members to coordinate care and achieve clinical cost reduction.
  • Identify and address barriers to care, including social, financial, and environmental factors that impact patient outcomes.
  • Engage patients, families, and healthcare teams in the transition plan of care to achieve better alignment with goals, department priorities, and system quality initiatives.
Requirements
  • Permanent LSW licensure is required.
  • LCSW licensure is preferred.
  • A Master's Degree in Social Work is required.
  • Relevant healthcare, mental health, or community-based experience is required.
  • Strong clinical assessment, planning, and goal development skills are required.
  • Ability to communicate and resolve conflict with diverse stakeholders is required.
  • Knowledge of community resources and entitlement programs is required.
  • Knowledge of state and county adult and child protective legal mandates is required.
  • Knowledge of the principles of growth and development over the life span is required.
