The RN Case Manager in our Care Coach Connect program is responsible for coordinating patient care and conducting initial and ongoing nursing assessments to address the needs of patients and their families. This role involves collaboration with interdisciplinary team members and the supervising clinician to ensure a comprehensive care plan.

Key Responsibilities:

  • Perform in-home medical assessments for established Care Coach Connect patients, ensuring thorough evaluations at each visit.
  • Conduct urgent visits to prevent hospital admissions, facilitating optimal care within the home environment.
  • Continuously assess the effects of chronic illnesses on patients' physical, functional, psychosocial, and environmental needs.
  • Evaluate caregiver stress and provide community resources as needed.
  • Implement and modify individualized care plans based on patient assessments.
  • Administer necessary procedures, including wound care, phlebotomy, and injections.
  • Engage in advanced care discussions with patients and families, documenting their preferences in the electronic medical record.
  • Educate patients and families about disease management, pain relief, end-of-life care, and ethical considerations.
  • Initiate preventive and rehabilitative nursing interventions when appropriate.
  • Maintain ongoing communication with patients and families throughout their care journey.
  • Evaluate patient and caregiver responses to care, adjusting plans as necessary.
  • Participate actively in interdisciplinary meetings, contributing valuable insights.
  • Utilize problem-solving skills to identify and implement effective solutions based on established practices.
  • Conduct telephonic follow-ups and case management as required.
  • Serve as a resource for LVN/MA staff.
  • Engage in continuous education to stay informed about chronic medical conditions and comply with licensing standards.
  • Build trusting relationships with patients, caregivers, clinic staff, and providers.
  • Collaborate with providers to recommend policies and standards that enhance patient care.
  • Demonstrate professionalism and courtesy in all interactions.
  • Communicate patient needs and outcomes to providers as per established protocols.
  • Coordinate services and prioritize patient needs with the interdisciplinary team.
  • Document progress toward care goals in the electronic health record.
  • Utilize a case management approach, referring to additional services as necessary.
  • Manage a patient caseload, ensuring timely visits and documentation.
  • Perform other nursing duties as assigned.

Required Qualifications:

  • Active Registered Nurse license in Texas.
  • Current BLS certification or ability to obtain within 30 days of hire.
  • Minimum of 3 years' experience in a clinical, hospice, or hospital setting.
  • Demonstrated nursing expertise and excellent communication skills.
  • Ability to work effectively with diverse teams.
  • Proficient in Microsoft Office Suite.
  • Strong organizational and prioritization abilities.
  • Valid driver's license and reliable transportation for travel.
  • Compliance with health screening requirements.

Preferred Qualifications:

  • Certification in Hospice and Palliative Care Nursing.
  • Two or more years of hospice experience.
  • Experience conducting home visits.
  • Knowledge of palliative and hospice care principles.
  • Experience in advanced care planning discussions.
  • Familiarity with community-based nursing practices.
  • Bilingual proficiency in English and Spanish.
