Job Overview

Service Coordination, Inc. is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and advocacy to individuals with disabilities, ensuring they receive the necessary resources and services to achieve their goals.

Key Responsibilities
  • Develop and implement Person-Centered Plans (PCPs) that outline individualized goals and objectives
  • Conduct comprehensive assessments to identify service needs and develop strategies for meeting those needs
  • Collaborate with individuals, families, and community resources to ensure seamless service delivery
  • Monitor and evaluate the effectiveness of services and make adjustments as needed
  • Provide education and support to individuals and their families to promote self-advocacy and independence
Requirements
  • Bachelor's degree in a human services field or related field
  • 2+ years of experience working with individuals with disabilities
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
