Job Summary

We are seeking a highly skilled and compassionate Bilingual Advanced Practice Clinician to join our Senior Community Care team at UnitedHealth Group Inc. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our most vulnerable populations.

Key Responsibilities
  • Conduct comprehensive assessments to identify the medical, social, and functional needs of our patients.
  • Develop and implement individualized care plans to address the complex needs of our patients, including those with chronic conditions and advanced illnesses.
  • Manage acute medical situations and exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Collaborate with the interdisciplinary care team to ensure seamless care coordination and communication.
  • Provide advanced illness and end-of-life discussions with patients and their families.
  • Document medical information accurately and completely, including ICD-10 conditions.
Requirements
  • Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Board certification in one of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Bilingual in Spanish.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
  • Access to reliable transportation.
Preferred Qualifications
  • 1+ years of clinical experience in a practice setting, preferably in long-term care.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
