About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) Case Manager to join our interdisciplinary team at WelbeHealth. As a key member of our care management team, you will play a vital role in delivering high-quality, patient-centered care to our vulnerable seniors.

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough nursing assessments to determine the care needs, preferences, and goals of our participants, and actively participate in interdisciplinary team meetings to develop personalized care plans.
  • Develop and Implement Care Plans: Deliver and document nursing interventions as agreed upon in the participants' care plans, including maintaining a safe and healthy environment, responding to physician orders, and administering medications.
  • Coordinate Care Transitions: Work closely with our WelbeHealth PACE Physician and other members of the interdisciplinary team to coordinate care needs and ensure seamless transitions.
  • Provide Education and Support: Instruct participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies to promote optimal health outcomes.
Requirements
  • Education and Licensure: Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred. Unencumbered California (RN) License required.
  • Experience: Two (2) years of nursing experience in a clinical setting with a frail or elderly population, preferred.
  • Certifications: Current Basic Life Support (BLS)/First Aid Certification required.
