Job Summary

Hydro Recruiting is seeking a highly skilled Senior Clinical Care Provider to join our team. As a key member of our interdisciplinary team, you will work closely with physicians and other healthcare professionals to provide comprehensive care to our clients.

Key Responsibilities:

  • Conduct comprehensive assessments and develop care plans for each participant.
  • Collaborate with physicians and the IDT to ensure effective care coordination.
  • Oversee and ensure effective care coordination in collaboration with physicians, community-based PACE physicians, and the IDT.
  • Participate in rotating night and weekend call duties.

Requirements:

  • Graduation from an Accredited School of Nursing.
  • Certification as a Nurse Practitioner with Prescriptive Authority.
  • NP State License.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care/geriatric issues and best practices.
  • Valid State Drivers License with a clean DMV record.
