As a Hospice RN Case Manager with Interim HealthCare, you will play a vital role in providing exceptional hospice care to patients and families in Charleston, Berkeley, and Dorchester Counties. Key Responsibilities:- Maintain a specified case management load, ensuring timely and effective care for patients and families.
- Conduct comprehensive assessments of patients' physical and psychosocial status, identifying needs and developing individualized plans of care.
- Collaborate with the Interdisciplinary Group (IDG) to coordinate care, ensure continuity, and achieve the goals of hospice care.
- Direct and supervise the services provided by LPNs, Hospice Aides, and Homemakers, ensuring high-quality care and patient satisfaction.
- Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, as indicated by patients' status and needs.
- Graduate of an accredited school of nursing with a current RN license in South Carolina.
- Twelve months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
- One year of hospice care experience, with precepting or training experience preferred.
- CPR certification and a valid driver's license with auto liability insurance.
- Competitive pay, with a salary range of $80K to $86K DOE.
