Position Summary: The Care Management Specialist is responsible for orchestrating comprehensive care delivery throughout the healthcare continuum to enhance clinical outcomes, minimize admission risks, and optimize the use of healthcare resources within the Health Network. This role collaborates with the care team, external partners, and patients to facilitate smooth transitions across various care environments. Acts as a patient advocate to coordinate post-acute services and ensure continuity of care.
Key Responsibilities:
- Facilitates post-acute services for patients in outpatient settings.
- Conducts warm hand-offs between different care functions and locations.
- Identifies patients requiring care coordination for transitions and wellness initiatives.
- Engages in interdisciplinary team discussions.
- Ensures patients comprehend their care plans and addresses any questions.
- Manages patient cases and executes action plans.
- Advocates for patients and informs the community about available services.
- Records care management activities meticulously.
- Communicates changes in patient conditions to Case Managers.
- Assists with special initiatives and mentors new Care Management Specialists.
- Performs assigned case management and operational tasks.
- May involve local travel to various sites.
Required:
- Graduate from a state-recognized Vocational Nursing program.
- A minimum of 4 years of LVN experience across diverse healthcare environments.
- Current LVN licensure from the Board of Nurse Examiners for the State of Texas or proof of reciprocity.
- 4+ years of experience in Case Management or as an LVN in healthcare settings.
