This role utilizes advanced technology to facilitate both real-time and asynchronous interactions with patients, broadening the scope of patient monitoring, education, and support throughout their healthcare experiences. Close collaboration with patients, caregivers, agency clinicians, and other healthcare professionals is essential to ensure continuity in the overall care plan.
Key Responsibilities
- Evaluates and monitors patients' health conditions using telehealth technology in their homes, along with feedback collected through surveys and virtual visits.
- Identifies changes or patterns in a patient's health status, taking appropriate actions based on established protocols and the patient's condition.
- Employs proactive patient management strategies, leveraging clinical expertise and critical thinking to implement evidence-based practices, minimizing the risk of complications.
- Coordinates with physicians/providers to address patient concerns, documenting orders in medical records and ensuring timely communication with agency staff for seamless care.
- Enhances the physician/patient/agency relationship by communicating effectively with healthcare professionals to address patient needs.
- Delivers thorough education to patients and caregivers via phone or virtual platforms regarding virtual care principles, equipment usage, disease management, and safety protocols.
- Encourages and empowers patients to adopt self-management techniques for chronic conditions, fostering active participation in their healthcare journey.
- Instructs and guides family members or caregivers in patient care and maintaining a safe environment.
- Responds promptly and flexibly to urgent patient needs as requested by the agency.
- Actively engages in case discussions at assigned agencies.
- Serves as a knowledgeable resource on diagnosis-specific best practices for agency staff and colleagues.
- Maintains current knowledge and educates peers on the administrative aspects of virtual care, including telemonitoring concepts, patient selection, technology use, and infection control measures.
- Supervises virtual patient care delivered by LPNs/MAs or non-clinical team members, ensuring compliance with regulatory standards.
- Assists in onboarding new team members as needed.
- Meets assigned productivity standards.
- Tracks personal performance and quality metrics, compiling data for leadership review to assess the effectiveness of the virtual care program in reducing hospitalizations and improving patient satisfaction.
- Exhibits strong oral communication skills when interacting with patients, performing triage, or collaborating with healthcare professionals.
- Demonstrates excellent written communication skills in documenting all interactions accurately and promptly, in line with organizational standards.
- Maintains professional licensure through ongoing education and training as required.
- Shows self-motivation and the ability to work independently with minimal oversight.
- Consistently upholds the organization's standards of behavior.
- Embodies a commitment to patient advocacy, exceptional customer service, integrity, and exemplary nursing practice.
- Performs other related duties as assigned within the scope of the position.
Licensure Requirements:
- Valid Registered Nurse license in the state of residence, including compact licensure; ability to obtain licensure in additional states as necessary.
- Current Driver's License, vehicle insurance, and access to reliable transportation.
- A minimum of three years of hands-on clinical experience as a Registered Nurse is required; five or more years of clinical experience is preferred.
- Prior experience in home health, ICU, ED, or similar high-skill environments is strongly preferred.
- Experience with geriatric, cardiac, pulmonary, and/or diabetic patient populations is highly desirable.
- Familiarity with remote telehealth monitoring or virtual care delivery is advantageous.
- Exceptional communication and customer service skills are essential.
- Proficiency with technology is critical, including electronic medical record documentation and various patient care software platforms.
