The Healthcare Service Coordinator is integral to the management of care and healthcare services for our Medicare Advantage and ACO patient demographics within our primary care facility. This role involves close collaboration with healthcare professionals to ensure effective transitions of care and adherence to treatment protocols. The Coordinator's responsibilities encompass direct patient interactions, facilitating clear communication, and providing support throughout the patient's healthcare experience. A thorough understanding of clinical care processes, combined with excellent communication skills, is essential to enhance patient satisfaction and improve care quality. This position is based in a clinical environment.

Key Responsibilities:

Care Management Coordination:

  • Work alongside clinical teams to guarantee comprehensive care for all patients, including pre-visit preparations and post-visit documentation.
  • Engage patients in health enhancement initiatives and educate them on self-management strategies to prevent unnecessary emergency department visits and hospital admissions.
  • Facilitate weekly team meetings to analyze population health data, close care gaps, identify high-risk patients needing visits, and review utilization patterns.
  • Proactively manage a panel of patients with chronic conditions, anticipating their needs and addressing obstacles to care.
  • Collaborate with patients, physicians, and care team members to evaluate progress towards health objectives and ensure accurate documentation of patient self-management efforts.
  • Uphold confidentiality and comply with HIPAA regulations.
  • Treat all individuals with dignity and respect.
  • Engage in professional development opportunities.
  • Participate in regional and market-level Patient Care Coordination and Population Health meetings to ensure local practice standards are met.
Coordination of Care Services:
  • Partner with healthcare providers and staff to identify patients who require care transition services.
  • Assist providers in referring high-risk, complex patients to the Complex Care Management program.
  • Collaborate with Population Health Specialists, Medical Assistants, and clinicians to address care gaps during patient appointments.
  • Oversee medication refills and work with the central Medication Adherence team to ensure patients with chronic illnesses adhere to their prescribed treatments.
  • Conduct outreach to patients to address care gaps, medication adherence, and frequent emergency department visits.
  • Coordinate referrals and manage utilization events according to established protocols and Standard Operating Procedures (SOP).
  • Monitor various platforms to stay updated on patient utilization events such as hospital admissions and emergency department visits, documenting occurrences.
  • Perform and document transitional care activities, including follow-ups post-hospital visits, medication reviews, obtaining hospital records, and scheduling TCM visits.
  • Coordinate consults, referrals, prior authorizations, and follow-ups with community resources.
  • Review and manage home health certifications and discharges with relevant providers to ensure appropriate service utilization.
  • Maintain accurate and current Inpatient (IP), Emergency Room (ER), and Home Health (HH) trackers per Standard Operating Procedures.
  • Coordinate with the ACO Clinical Team to escalate issues with ACO members within the practice's panel.
  • Ensure that patients' health plans are appropriately flagged in the EMR, including new ACO members.
  • Manage medication prior authorizations to facilitate timely patient access to prescribed therapies.
  • Provide clinical follow-up with patients as necessary and offer guidance on effective care transitions.
  • Support the use of coding tools in the clinic during patient visits to accurately capture chronic conditions.
  • Maintain precise and timely documentation of care coordination activities.
  • Engage in measuring clinical outcomes and performance improvement initiatives.
  • Offer backup support for medical assistant services within the clinic as needed, utilizing a collaborative approach.
Qualifications:
  • Preferred: Active Medical Assistant Certification.
  • Minimum: One (1) year of medical experience in a physician's office.
  • Experience in Primary Care is highly desirable.
  • Familiarity with value-based care and Medicare Advantage is preferred.
  • Bilingual candidates are preferred.
Knowledge, Skills, and Abilities:
  • Current Basic Life Support Certification.
  • Advanced knowledge of medical terminology.
  • Support for the practice's mission and goals.
  • Bilingual Spanish may be preferred.
Physical Requirements:
  • Carrying/Lifting: Minimal.
  • Standing: Up to 8 hours per day.
  • Walking: Up to 8 hours per day.
  • Repetitive Motion: Required to work at a keyboard for up to eight hours per day.
  • Visual Acuity: Required to view a computer monitor and read printed materials with or without corrective lenses for up to eight hours per day.
