Job Summary


We are seeking a skilled and compassionate Healthcare Navigator and Transportation Coordinator to join our team. In this role, you will provide vital navigation services to patients enrolled in our Community Mental Health Grant Program.



About the Role


As a Healthcare Navigator and Transportation Coordinator, you will work closely with our interdisciplinary team to ensure our patients receive the care they need. Your primary responsibilities will include:



  • Providing field-based navigation services to address the social determinants of health within our patients.
  • Working with our team to develop and implement comprehensive care plans for our patients.
  • Coordinating transportation services to ensure our patients can access the care they need.
  • Maintaining accurate records and reporting on patient outcomes.
  • Collaborating with community partners to expand our services and improve patient outcomes.


Requirements


To be successful in this role, you will need:



  • A Bachelor's degree in Social Work, Sociology, Psychology, Health Education, or a related field.
  • At least 3 years of experience in a healthcare setting, preferably in a navigation or coordination role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects.
  • Valid Texas driver's license and a reliable vehicle.


Preferred Qualifications


Bilingual in English and Spanish is a plus, as is experience working with diverse populations and in community-based settings.



What We Offer


We offer a competitive salary of $45,000 - $55,000 per year, depending on experience, as well as a comprehensive benefits package, including health, dental, and vision insurance, and a 401(k) retirement plan.
