Job Summary

The Clinical Social Worker - LBSW will manage the care for a specific group of patients, facilitating the safe movement of patients throughout the continuum of care. This role ensures optimal utilization of resources, service delivery, and compliance with external review agencies, local, state, and federal requirements.

Key Responsibilities
  • Perform comprehensive psychosocial assessments to identify patient health goals and coordinate services with the RN Case Manager for an effective transition plan.
  • Meet directly with patient/family on an ongoing basis to address social/psychosocial issues.
  • Develop, coordinate, implement, and revise a care plan to ensure good transition management and continuity of care, cost reduction, and patient self-management.
  • Collaborate with the physician and multidisciplinary team to facilitate care and assist in meeting discharge goals.
Requirements
  • Bachelor's degree in Social Work (BSW) from an accredited school.
  • Minimum 2 years of clinical experience as a social worker in a medical/clinical setting.
  • Excellent interpersonal communication and negotiation skills.
  • Ability to identify and coordinate appropriate resources in the community.
  • Computer skills.
