We are seeking a skilled Social Worker to join our team at Transitions Care, a leading hospice organization dedicated to providing exceptional care to patients and their families.
Key Responsibilities- Conduct thorough psychosocial assessments of patients and their families to identify their needs and develop personalized care plans.
- Provide supportive counseling, problem-solving, and community referrals to meet the needs of patients and families.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and delivery.
- Maintain accurate and timely records of patient interactions and care provided.
- Bachelor's degree in Social Work, Master's degree preferred.
- Licensure and/or certification as required by the state.
- Minimum of 2+ years social service experience in a healthcare setting or social services agency.
- Strong knowledge of local community resources and ability to obtain referrals.
