Job Summary:

Liberty Health is seeking an experienced Registered Nurse to join our Home Health Care team. As a Home Health Care Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Responsibilities:

  • Participate in the interdisciplinary care planning process to ensure comprehensive patient care.
  • Provide professional nursing services in accordance with the plan of care established by the Patient Care Coordinator.
  • Promote favorable patient outcomes through collaborative practice patterns and the effective use of resources.
  • Accurately and thoroughly document patient care to demonstrate evidence of delivered services, facilitate interdisciplinary coordination, justify reimbursement, and support quality improvement activities.
  • Develop and maintain positive relationships with patients, families, and healthcare team members.
  • Take personal accountability for accomplishing work assignments and contributing to professional growth.

Requirements:

  • Successful completion of nursing education from an accredited school of nursing.
  • Current RN licensure in the state where the branch is located, with at least one year of clinical nursing experience in an acute care or long-term setting, and home health experience preferred.
  • Current knowledge of fundamental medical-surgical nursing principles and practice.
  • Excellent clinical nursing skills and ability to communicate effectively, both verbally and in writing.
  • Ability to use a computer and have a working knowledge of various software applications.
  • Possess high-level problem-solving, critical thinking, and reasoning skills for use in patient care planning and problem resolution.
  • CPR certification.
