Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality care to our residents, working closely with our healthcare team to ensure their physical, emotional, and social needs are met.

Key Responsibilities
  • Provide direct care to residents, including administering medications, monitoring vital signs, and assisting with daily living activities.
  • Develop and implement individualized care plans, working closely with residents, families, and healthcare professionals.
  • Conduct regular assessments and evaluations to identify resident needs and make recommendations for care.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Participate in training and education programs to enhance skills and knowledge.
Requirements
  • Current LPN license in applicable state.
  • CPR certification.
  • Ability to lift 50 lbs. and stand for long periods.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
