Home Health Care Professional

As a Home Health Care Professional with All Ways Caring HomeCare, you will provide high-quality, compassionate care to clients in their homes. Your primary responsibility will be to deliver personalized care services that meet the unique needs of each client, promoting their dignity, independence, and overall well-being.
Key Responsibilities:
Provide 1:1 care to clients, following their individualized plan of care and maintaining their dignity and independence
Monitor clients' vital statistics and report any abnormal findings to the case manager and physician
Administer medications, feedings, oxygen, and ostomy care as needed, and document the results
Complete routines established by other care professionals, such as Physical Therapists, Occupational Therapists, and Speech Therapists
Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections
Accurately document nursing actions, communicate with family, case manager, and physicians, and document progress and outcomes for established goals
Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand
Ensure all patient needs are met in accordance with the instructions of the physician
Monitor incidents involving individuals and ensure incident reports are completed timely and accurately
Requirements:
Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
Current Cardiopulmonary resuscitation certification
Ability to work in a home-like setting
Ability to bend, stoop, push, pull, sit, and walk for periods of time
Ability to utilize proper body mechanics while lifting 50 pounds
Knowledge of individual service plans, including understanding of client rights and health needs
Must have completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
