RN Case Manager Home Health Full Time

Your Key Responsibilities- Develop and implement comprehensive care plans for patients, ensuring their unique needs are met.
- Collaborate with our interdisciplinary team to provide seamless care coordination and ensure patients receive the best possible outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure a smooth care experience.
- Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Supervise and mentor home health aides and other care team members to ensure high-quality care delivery.
- Stay up-to-date on industry developments and best practices to continuously improve patient care.
- A Diploma, Associate, or Bachelor's Degree in Nursing.
- A minimum of one year of nursing experience, preferably in home health or a related field.
- Strong med-surg, ICU, ER, and acute care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Excellent organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
