Your Key Responsibilities
  • Develop and implement comprehensive care plans for patients, ensuring their unique needs are met.
  • Collaborate with our interdisciplinary team to provide seamless care coordination and ensure patients receive the best possible outcomes.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure a smooth care experience.
  • Monitor patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
  • Supervise and mentor home health aides and other care team members to ensure high-quality care delivery.
  • Stay up-to-date on industry developments and best practices to continuously improve patient care.
What We're Looking For
  • A Diploma, Associate, or Bachelor's Degree in Nursing.
  • A minimum of one year of nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Excellent organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
