Job Summary

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at Santa Rosa Post Acute. As an LVN, you will play a vital role in providing high-quality care to our residents, ensuring their comfort, safety, and well-being.

Responsibilities
  • Assist residents in achieving their highest level of independence and self-care.
  • Make rounds and provide report to ensure seamless care delivery.
  • Participate in orientation and ongoing training of staff to maintain a high level of care.
  • Chart nurses' notes in a clear and descriptive manner, reflecting the care provided and the resident's response.
  • Respond to and monitor care issues and changes in condition, communicating with physicians and other healthcare providers as needed.
  • Supervise and instruct nursing assistants in provision of care, ensuring prompt response to call lights.
  • Assist physicians and other healthcare providers during visits and examinations.
  • Communicate new or updated orders for rehabilitation therapies.
  • Provide resident care, including administering medication and treatment, as per physician's orders.
  • Report significant findings or changes in condition and potential concerns to the Director of Nursing.
  • Protect residents' rights, privacy, and property.
Requirements
  • Current California LVN license.
  • 1 year of nursing experience, preferably in long-term care or a related field.
