Physical Therapist Assistant

This position is located in the home care referrals center, on site in the Garden City office. The schedule requires rotating weekends and holidays.
As a Physical Therapist Assistant, you will assist in administering physical therapy care programs, monitor and communicate patient progress to Physical Therapists and other medical/allied health personnel, and maintain medical records for each patient.
Key Responsibilities- Administer direct patient care in accordance with the written and signed prescription of the referring physician or other State authorized practitioner, as directed.
- Submit required documentation in accordance with the policies and procedures of the department.
- Maintain progress notes and discharge summaries of assigned patients.
- Respond appropriately to established department emergency procedures for patient safety and care when necessary.
- Participate in quality improvement chart reviews and activities.
- May attend family meetings/conferences pertaining to the welfare and management of assigned patients.
- Perform related duties as required.
- Associate's Degree required, or equivalent combination of education and related experience.
- Current License to practice as a Physical Therapy Assistant in New York State required, plus specialized certifications as needed.
- 1-3 years of relevant experience required.
