Key Responsibilities:

  • Develop and implement individually designed therapy programs to restore function and independence in the home setting.
  • Collaborate with physicians, family members, and other healthcare professionals to ensure effective patient care.
  • Provide timely and accurate documentation of visits and communicate discharge plans with family members and team members.

Requirements:

  • Home health experience is preferred but not required; we offer mentorship and support for new hires.
  • Ability to accept patients within your geographical area of coverage.
  • NY Physical Therapist License or eligibility.
