Summary/Objective
In alignment with our mission to enhance the lives of our Guests, the Social Worker is responsible for addressing the emotional and social needs of Guests on an individual basis, ensuring compliance with applicable federal, state, and local regulations.
Key Responsibilities

1. Maintain current evaluation documentation for each Guest's activities in accordance with regulatory standards.
2. Create a detailed social history and psychosocial assessment that captures the Guest's challenges, strengths, and preferences, considering cultural, religious, and ethnic backgrounds.
3. Guide Guests and their families through the facility's services, limitations, and rights.
4. Participate in care conferences, identifying psychosocial needs for inclusion in care plans, particularly focusing on treatment schedules and environments that promote individuality.
5. Assist with the coordination of intake, admissions, and transfers within the facility.
6. Discuss facility regulations with families and address any inquiries regarding these policies.
7. Oversee the inspection of Guests' belongings to ensure proper labeling and inventory.
8. Attend interdisciplinary team meetings, assess Guests for psychosocial groups, and supervise documentation processes.
9. Complete required documentation within the timelines set by regulations.
10. Provide regular consultations to Guests to address issues and support the implementation of solutions.
11. Mediate conflicts that arise among Guests, families, and staff.
12. Connect Guests with appropriate community resources by maintaining awareness of available services and identifying unmet needs.
13. Collaborate with Guests, families, and multidisciplinary staff to develop comprehensive discharge plans.
14. Offer informal counseling to help uncover issues that may hinder Guests' socialization and participation in activities.
15. Establish and maintain an organized filing system for the Social Services Department.
16. Report hazardous conditions and equipment issues to the appropriate personnel.
17. Adhere to established safety protocols while performing tasks and using equipment.
18. Ensure the comfort, privacy, and dignity of Guests, interacting with them in a respectful and caring manner.
19. Communicate effectively and tactfully with Guests, visitors, families, peers, and supervisors.
20. Respond promptly and courteously to call lights in Guest care areas.
21. Attend departmental meetings and training sessions as required.
22. Recognize and fulfill your role in the organization's Quality Assurance and Performance Improvement (QAPI) initiatives.
23. Participate in training to enhance understanding and capacity for QAPI work.
24. Execute assigned QAPI roles and responsibilities.
25. Follow established policies and procedures to support QAPI efforts.
26. Share improvement ideas with leadership.
27. Communicate with leadership regarding positive outcomes from QAPI projects or barriers to success.
28. Support QAPI initiatives through actions and attitudes, adjusting practices as necessary.
29. Participate in compliance training and adhere to the compliance plan.
Supervisory Responsibility
This position does not have supervisory responsibilities.
Work Environment
This role operates within a healthcare setting, requiring regular movement throughout the facility and the use of standard office equipment.
Physical Demands
This position is primarily sedentary but may require some filing, necessitating the ability to lift files and open cabinets.
Position Type and Expected Hours of Work
This role is part of a healthcare facility that operates 24/7, requiring flexibility for evening, weekend, and holiday shifts.
Travel
This position may involve travel to various locations within the surrounding community.
Required Education and Experience
· Minimum of two years' experience in a social worker role, preferably in a healthcare environment.
Preferred Education and Experience
· Bachelor's degree in social work or a related field.
· Five years of relevant experience.
