Key Responsibilities:
The Healthcare Case Manager will play a crucial role in facilitating services that empower patients to achieve optimal health outcomes. Responsibilities include:
- Conducting comprehensive assessments to identify patient needs.

- Coordinating access to healthcare services and community resources.
- Advocating for patients to ensure they receive appropriate care.

Qualifications:
