MDS Care Coordinator

California Terrace is seeking a highly skilled Care Plan Coordinator to join our team. The successful candidate will be responsible for ensuring the accuracy of resident care plans and facilitating effective communication between residents, families, and healthcare staff.
Responsibilities:
- Coordinate the completion of the Minimum Data Set (MDS) upon admission, readmission, and quarterly and annually reviews.
- Provide monthly and as-needed schedules of scheduled MDS assessments and care plan conferences.
- Assure timely completion of each MDS section by the appropriate department and sign off on the form by staff in the electronic medical record (EMR).
- Review diagnosis coding and sequencing with physicians quarterly and update/clarify ICD-9 coding as necessary.
- Conduct weekly MDS pre-planning and Medicare meetings to ensure timely and accurate completion.
Requirements:
- A current State of Illinois license as a Registered Nurse or Licensed Practical Nurse.
- Ability to read, write, and speak English in an understandable manner.
- AANAC certification preferred.
Working Conditions:

- Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing, the MDS, and its computer application (EMR).
