Job Summary

The Community Health Navigator plays a vital role in supporting the social, economic, and behavioral health needs of vulnerable patients within the Maui Health System and the communities we serve.

This position requires a highly trained communicator and expert in accessing local community resources and social services. The ideal candidate will be skilled in motivational interviewing and person-centered care, with the ability to identify goals and enable members and their families to achieve those goals through identifying local community assets.

The Community Health Navigator will work on inter-disciplinary care teams to support members who are experiencing complex socio-economic circumstances impacting their health and healthcare. They will coordinate alternative services for patients with social and non-medical needs, where other avenues of care are more appropriate to affect optimal patient outcomes, achieve continuity and quality of care, and reduce costs.

Essential Responsibilities
  • Supports members and their families who are experiencing complex circumstances.
  • Works to identify goals and enable members and their families to achieve those goals through identifying local community assets, vetting their services, and making these resources accessible to patients and providers.
  • Coordinates alternative services for patients with social and non-medical needs.
  • Collaborates with inter-disciplinary care teams to support members with complex socio-economic circumstances.
Requirements
  • Minimum one (1) year of experience in a healthcare or related field.
  • Bachelor's degree in a business or healthcare-related field, or four (4) years of experience in a directly related field.
  • Competent computer skills with proficiency in Microsoft Office products and ability to learn new programs to support the program.
  • Ability to demonstrate flexibility and adaptability when faced with internal or external barriers, or when faced with differing points of view.
  • Ability to use integrated technology platforms and virtual care coordination tools.
