Job Summary:
The Foster Care Case Manager will be responsible for facilitating the return home of children to their biological parents or finding a forever family through adoption or guardianship. This role requires strong communication and interpersonal skills, as well as the ability to work with a variety of clients, families, foster parents, and adoptive parents.
Key Responsibilities:
- Visit regularly with children and their biological parents to assess their needs and develop a plan for reunification or alternative care.
- Work with clients, families, foster parents, and adoptive parents to ensure all necessary services are provided.
- Maintain accurate and timely documentation of client interactions and outcomes.
- Collaborate with other professionals to ensure a comprehensive approach to client care.
- Participate in on-call rotation to provide emergency support to clients and families.
Requirements:
- Bachelor's degree in social work, education, or another field in human services and 2 years of experience in the provision of foster care or other out-of-home care.
- Master's degree in social work, education, or another field in human services and licensure by DCFS or eligibility for licensure.
- Ability and willingness to complete the children and residential experiences (CARE) and therapeutic crisis intervention (TCI) training.
- Valid driver's license and proof of insurance.
- Must be 21 years of age.
