Job Summary:

The Home Health Physical Therapist will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This includes evaluating patients, developing and implementing individualized plans of care, and providing ongoing therapy services to promote optimal patient outcomes.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patients to determine their physical therapy needs and develop individualized plans of care.
  • Implement and update plans of care as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document all aspects of patient care, including evaluations, treatments, and progress notes.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Ability to effectively communicate and create positive impressions with patients, families, and healthcare teams.
  • Knowledgeable on therapy best practices.
  • Ability to make appropriate physical therapy judgments.
  • Ability to identify a situation and handle it with the best possible solution.
  • Detail-oriented and observant.
  • Disciplined style of work ethic with the ability to prioritize and be timely.
  • Valid driver's license and auto insurance with your name as a listed driver.
