Position Summary:

This role requires proficiency in Spanish.

The official title for this position is Lead Care Coordinator (LCC).

The Lead Care Coordinator (LCC) is responsible for creating tailored care plans, managing member services, and working closely with members and their families to ensure they receive necessary medical care and support. The LCC will engage with members to assess their needs, formulate personalized action plans, and collaborate with care teams to effectively manage the member experience. Key responsibilities include providing emotional support, addressing administrative challenges, and ensuring timely access to healthcare services. Compassion, healthcare expertise, and outstanding customer service are essential attributes for helping members achieve health independence.

Key Responsibilities:

  • Conduct interviews with members to evaluate medical and social needs while providing education about their conditions and medications, and developing personalized care plans.
  • Address member inquiries and concerns, ensuring compliance with healthcare regulations.
  • Collaborate with interdisciplinary teams, identify medical and social resources, and coordinate social service plans.
  • Advocate for members, liaise with healthcare providers, schedule appointments and treatment plans, monitor member progress, and assist with healthcare obstacles.
  • Maintain empathy and professionalism in communications with members and families.
  • Support behavioral health coordination, substance abuse resources, and community services.
  • Perform additional duties as assigned.

Qualifications:

  • Valid driver's license and vehicle insurance.
  • High school diploma or GED. (Required)
  • Minimum of 1 year of experience in case management, member care, customer service, or related fields. (Required)
  • Medical Assistant certification or knowledge of medical terminology. (Preferred)
  • Strong communication, interpersonal, customer service, and organizational skills.
  • Proficient in documentation, email, and chat support.
  • Able to work independently and collaboratively in a team setting to provide member care.
  • Excellent multitasking, organizational skills, and attention to detail are essential for managing care for multiple members simultaneously.
  • Ability to effectively utilize resources for problem-solving while maintaining composure under pressure in a compassionate manner.
  • Strong speaking, reading, and typing abilities, along with proficient proofreading skills.

Work Environment / Physical Requirements:

  • Must be able to travel using a personal vehicle for outreach visits.
  • Prolonged periods of sitting at a desk using a computer.
  • Ability to lift up to 15 lbs.

Travel Requirements:

  • Initial training will be provided, with travel expenses covered.
  • Meal stipends will be provided during training.
