Key Responsibilities:
  • Coordinate the plan across the care continuum through critical thinking, teamwork, and communication between care providers, patients, families, and external vendors.
  • Prioritize and coordinate the plan to ensure timely discharge.
  • Work effectively in a fast-paced environment with rapidly shifting priorities and competing demands.
  • Exercise discretion and prioritize tasks, seeking input as indicated.
Requirements:
  • HS Diploma or General Education Diploma (GED)
  • 1-year recent relevant experience
  • Oral and written communication skills
  • Interpersonal and time management skills
  • Ability to work independently with a minimum of direction
  • Intermediate PC skills and word processing skills required
