Job Title: Community Living Support SpecialistAbout the Role

We are seeking a skilled and empathetic Community Living Support Specialist to join our team at PACE Southeast Michigan. As a key member of our Community Living Program, you will provide exceptional support and care to our participants in their homes.

Responsibilities
  • Provide supportive services, including grocery shopping, laundry, and household tasks, as outlined in individual participant care plans.
  • Assist with daily living activities, such as meal preparation, medication management, and toileting.
  • Engage participants in recreational activities, promote socialization, and encourage independence.
  • Maintain accurate and timely documentation of participant care and services provided.
  • Respond to emergency situations and follow established policies and procedures.
Requirements
  • High School diploma or equivalent required.
  • Valid Michigan driver's license and reliable transportation.
  • Current CPR certification required.
  • Certification as a nursing assistant or completion of a basic nursing assistant training program preferred.
Working Conditions

Work environment is variable, with potential exposure to communicable diseases. Must have ability to lift 40 pounds or more and transfer patients as needed.

The estimated salary for this role is $42,000 - $50,000 per year, depending on experience.
